As usual, the title was one of the hardest parts.  It's a reference to a segment that happens long after the scene this is taken from.  Mokuba is the younger brother of Kaiba Seto from the anime Yuugiou [Yu-Gi-Oh!].  The two are orphans and got adopted by a really nasty jerk.  From that time, Kaiba never smiled.  When confronted with their past, Mokuba says that if he can change the past he would rather have the brother that smiles all the time.

It seemed to fit the expression.  *shrugs*
